- Patient portion due day of service.
- Cannot be used with another plan.
- No refunds of membership payment.
- NON REFUNDABLE

Cannot use Care Credit or other financing options
Our Membership Plan Includes:
- 1 comprehensive exam
- 15% off emergency exam (used at any time during the year)
- 2 regular cleanings (non-periodontal)
- 15% off periodontal cleanings
- 2 oral cancer screenings
- 1 fluoride desensitizing treatment
- 4 bitewing x-rays
- 15% off minor services such as but not limited to dental sealants, fillings, or extractions.
- 17% off major services such as but not limited to implants, crowns, bridges, partials and dentures.
PROGRAM EXCLUSIONS & LIMITS
- In conjunction with another dental plan, dental insurance, or financing.
- For treatment which, in the sole opinion of our doctors, lies outside the realm of their capacity.
- For referrals to specialists.
- For hospitalizations or hospital charges of any kind.
- For cost of dental care which are covered under automobile medical. For services of injuries covered under automobile medical.
- This plan is only honored at Edgerly Dentistry and can not be used at any other office.
